Reading 400 pages of records to build a chronology is necessary and slow. Per-page chronology vendors make it faster and make every case more expensive.
A chronology without page cites cannot be trusted in a demand or at mediation.

Records come in
Filed records are read once, using the text layer when there is one and OCR when there is not.

Visits extracted
Date, provider, complaints, findings, diagnoses and procedures, with the source page for each line.

Gaps flagged
Treatment gaps and missing providers are highlighted, which feeds straight into the records board.

Review and export
Staff review and correct lines, then export to the demand package or as a standalone PDF.
Approval: The chronology is internal work product. Staff confirm it before it is used in a demand.

Is there a per-page charge?
No. Chronologies are included in the seat price.
Does it handle handwritten records?
OCR handles most typed and many handwritten pages. Pages it cannot read confidently are flagged rather than summarized.
Can we upload real patient records?
Yes, once your firm has signed our Business Associate Agreement in the app. Records are stored in Microsoft Azure Blob Storage under Microsoft's BAA and read by OCR and models running on Azure.
